We received a service inquiry from a wife whose husband had suffered a stroke. He was going to be discharged from the skilled nursing facility back to their home in North Raleigh. The wife was very concerned because her husband was not “back to normal." When I asked her about these concerns, she told me that he is not allowed to drive a car and she worried about his ‘thinking’ since the stroke. She also said the therapists wanted him to have a caregiver with him ‘all the time’.
Her husband had been very active, going out with friends, going to the gym to exercise, and he handled all of their finances. She said he would not want someone hovering around him and she was concerned about the cost. She said they had many friends in Raleigh and a strong support system with their church.
Assessment and Plan of Care:
After assessing the client, I assured both of them we could provide services to help them thrive in their home. We started with caregivers 24/7 as directed by the facility discharge, but were able to reduce services to 2 visits per day after one week. Our caregivers provide transportation for errands and appointments as well as help around the house. The caregivers learned the client’s routine and provided assistance with getting in and out of bed safely, bathing and dressing, as well as meal preparation. We built a consistent team of caregivers who also reminded the client to perform the exercises the Gentiva therapists ordered.
This teamwork led to the client becoming strong enough to reduce the caregiver visits to one per day. The client and his wife are thrilled to have the assistance he needs as well as meals prepared, the linens changed and the laundry done. Most of all, they are able to live together in the home they have loved for over 40 years.
